The parking is just East of the office on S.W. Blvd. one block west of Snow Glass on Main.

My Office hours are: 8:00 to 12:00 and 1:30 to 5:30 on Monday through Friday. Office hours are by appointment.

I am a preferred provider for many insurance companies. I accept assignment on most insurance.I am a Medicare provider and I accept Medicare assignment.

I accept cash, check, visa & mastercard as payment.

Emergency services are provided to existing patients who hurt them self's after hours and are in need of care by calling my cell phone or my home phone.

House calls in Blackwell only are provided to existing patients whose injury makes it impossible for them to come to my office.
